A friend and co-worker used to work for an AT&T subsidiary, designing and testing phone switches. He said that their switches used to be designed and tested and utterly reliable. After the breakup of the Bell System, they got many cheaper competitors, who shipped buggy switches, then made iterative partial fixes over time. The market moved to the cheaper products and AT&T followed.
