@1g0r same problem here (you need to use the webmaster tools (in Firefox for example) o see which error message you get).
When I think harder about it I think the explicit error was a cache miss, so maybe you could report it to !osm and copy the URL to the affected tiles. -

@1g0r I've had some issues with refreshing tiles but that was relatively temporary and I don't experience it now. (I think I got a "not here anymore" error message or something) -
